If you are 18 or over, you can conveniently complete your patient information on our new in-office electronic registration tablets. It takes only 10 minutes and your information will be securely stored in our central database for future visits.

If you are the parent of a child visiting us for the first time, please take a few moments to download, print, and complete the forms listed below prior to your visit. Bringing these completed forms to your appointment will help us to process your paperwork more efficiently, reducing the time spent waiting to see the dentist.
